South Side Help Center delivers Direct HIV Care with a compassionate, community‑rooted approach that meets people exactly where they are. Their team provides confidential testing, personalized counseling, linkage to medical treatment, and ongoing support that helps clients stay engaged in care. HIV Care
South Side Help Center’s HIV Care program connects clients to ongoing medical treatment, medication management, and personalized support that helps them maintain their health and stay engaged in care. Through its partnership with AIDS Healthcare Foundation Healthcare Centers, SSHC ensures every client receives high‑quality, compassionate services regardless of ability to pay, along with the community‑based guidance that strengthens long‑term wellness.
Case Management
South Side Help Center provides comprehensive case management services for people living with HIV, offering support with housing stability, medical and non‑medical care coordination, psychosocial support groups, and specialized services for individuals returning from correctional settings. These programs are designed to reduce barriers to care, strengthen overall wellness, and keep clients connected to the resources they need to thrive.
Housing clients are connected through the Coordinated Entry System, while all other case management referrals come through AIDS Foundation Chicago. Although SSHC does not conduct direct intake, the team is committed to ensuring every individual is guided to the right services and support networks.

How Clients Are Connected to Us
Housing Case Management: All housing clients are referred through the Coordinated Entry System, which connects individuals to housing resources based on need.
All Other Case Management Services: Clients are referred through AIDS Foundation Chicago. SSHC does not provide direct intake for case management.
